I did so much that I lost track. Okay, here's a list of my discrepencies and the corrections therewith:
-Omit "Protect'
-'Reflega' has an alternate option: Magic Coat + Spread
- 'Firaga', 'Blizzaga', and 'Thundaga' have a 25% chance of causing their status problems, not 49%.

Yeah, I did go just the slightest bit overboard... but I am too lazy to change it =P

"Death Scythe"

Effect: Can cause various effects: 10% chance of doing nothing; 1% chance of causing FNT; 4% chance of causing Toxic and Confusion; 5% chance of causing Toxic and SLP; 20%chance of causing SLP; 20% chance of Toxic; 20% chance of FRZ; 20% chance of PAR.
Flavour: An attack that call upon the user's inner reapr. The target is slashed with claws imbued with death and causes various effects.
HIDDEN EFFECT: Dispels the effects of Aqua Ring, Ingrain, and Healing Wish.
FIELD EFFECT: If used on a patch of grass where a Pokemon is hiding, a Battle is initiated with the wild Pokemon coming in with only 25% of its total HP. After the battle, the patch of grass is gone and will not grow back until you enter a building.

"Life Anthem"

Effect: The user sings a song filled with rejuvenating energy that restores health over time (Next Turn: 50% of total HP; 2 Turns Passed: 37.5% of total HP (rounded down in the true Nintendo fashion); 3 Turns Passed: 9.375% of Total HP (again, rounded down); 4-6 Turns passed: 2.34375% of total HP (round DOWN); 7+ Turns Passed: NADDA, ZIPPO, ZERO!!!. [Whew! The math got complicated, didn't it??]
Flavor: A song filled with love and life issues from the user and ehoes about them for 6 turns after it is used, getting weaker as time goes on.
HIDDEN EFFECT: Dispels the effect of Perish Song
FIELD EFFECT: Random Encounters restore 12.5% of Total HP at the start ("A lovely song seems to pierce through the roar of battle!" (NOTE: Only works for ONE random encounter! After that, you'll have to use it again in order to keep the effects going!)).
