Those problems could be easily explained...

Perhaps Torterra could shed their shells? Similar to the same way a Snake sheds it's skin. When they get a certain age and during the right times (Such as when Land Masses Shift) Their shells could dry out, crack, break and reveal a newer and up to date Shell. (There could be meeting ceremony between them where this happens. Similar to The Mysterious Garden Episode where all the Bulbasaur Meet to Evolve. As well as the way the Turtles of the world meet at the Galapagos to mate..)

As far as the Health Thing Goes. I don't think the condition of their Shell in relationship to Health should be that big of an issue. It think the map should be like our skin. If you cut yourself it heals over. Hair doesn't stop growing there or anything. Everything returns to the way it was. Meaning that once a Torterra returns to it's normal health the map should return to normal, but when they are sick, maybe the map is distorted or blurred or something.

Also I don't really like the idea of Globes being suspended on above their bodies on spikes. I think they should just have a World Map image imprinted on their Shells. As far as age goes. I think growing up Torterra should only have maps of places they have been or seen, but once they hit (maturity) they should be able to develop the world map on their shells. Or maybe just when they are little, the map should be too small to read until they are older...
